Before penicillin became widely available, sulfonamide drugs were the first effective antibacterial medications used on a mass scale. Every American GI carried sulfa powder in their first-aid kit—small packets of crystalline sulfonamide powder that was sprinkled directly into open wounds to prevent infection.

The practice of direct wound application was revolutionary. In previous wars, infected wounds were the primary killer of battlefield casualties. In World War I, the wound infection rate was approximately 80%; in WWII, with sulfa powder and later penicillin, it dropped dramatically. Medics were trained to tear open the packet and liberally dust the wound before applying bandages.

Sulfa drugs had limitations: they were ineffective against some bacteria, could cause kidney damage, and allergic reactions were common. As penicillin became available in quantity after D-Day, it gradually replaced sulfa as the primary battlefield antibiotic. Nevertheless, sulfa powder saved countless lives in the early war years, particularly in North Africa and the Pacific, and represented a fundamental revolution in military medicine.
